Insurance Frequently Asked Questions
I am in-network with Aetna, Blue Cross Blue Shield, United Healthcare, Cigna, Ambetter, Medicare, and United Healthcare Community Medicaid Plan (TennCare).
To comply with Tennessee’s requirements regardless of your insurance company, I will need a referral from your provider PRIOR to seeing you at your appointment. I have a referral form you can give to your provider HERE. I am also happy to fax it to your provider for you and obtain the referral for you.
**Note: If you have Medicare, the referral must come from a doctor (MD/DO). It cannot come from a nurse practitioner (APRN/NP) or physician assistant (PA).
I will call on your behalf to verify your coverage and let you know if there are any out-of-pocket costs you will be responsible for after speaking to your insurance company representative. However, I can only go by the information they told me. Ultimately, it is your responsibility to know if your insurance policy provides coverage for nutrition counseling and pay for the co-pay or cost of the visit if the insurance company does not cover the service.
Therefore, I HIGHLY recommend you also call them and ask about your nutrition counseling benefits PRIOR to your scheduled visit to confirm your coverage. I have put together a list of questions HERE to ask when you call the 1-800 number on the back of your card and speak to a rep from member services.
Most insurances are covering for telehealth services currently, however it is possible your insurance plan may impose a cost-share for you to use this service. I have included asking about telehealth coverage on the list of questions to ask the representative PRIOR to scheduling your visit under the tab, “What questions should I ask when calling my insurance company?”
I am out of network with any insurance provider not listed above. Some insurances may provide out-of-network coverage for your visit, therefore you should still call your insurance company PRIOR to your visit to confirm if your nutrition visits will qualify for any reimbursement. Payment for your visit is due upfront. I can then provide you with a superbill to submit to your insurance. Your insurance company will reimburse you depending on what your out-of-network benefits are.
Not every insurance policy has nutrition counseling as a benefit. If your specific insurance plan will not cover the sessions or you have a health insurance provider which I do not participate with, self-pay options are available. Self-pay options are also available if you do not want your insurance billed or you are without insurance.
My initial visits (60-minutes) are $120.00 and each follow-up visit (30-minutes) is $60.00. Payment options include credit/debit cards, health savings account (HSA) and flexible spending account (FSA) cards. Please note, you will be responsible for paying at the time of your visit.
